With over 50 years of combined experience, our founders assembled an experienced team of compassionate caregivers to change the way patients feel about their doctors, their health, and themselves.
Our Philosophy
Health is happiness, and being happy is all most of us want. Why, then, does going to the doctor fill so many of us with dread? Too many patients find their concerns dismissed, their symptoms disregarded, their experiences ignored by healthcare professionals. Patients too often feel judged, excluded from medical decisions, and dismissed by the very people they depend on to help keep them healthy.


At Alertive, we know that providers have a responsibility to listen to patients, to empower them to take care of their health, and to break down the barriers between patient and doctor. Our healthcare philosophy combines advanced medical technologies with a diverse, expert staff that puts the patient’s needs at the center of care. With a focus on community and family, we adapt our care to the needs of many different patient populations with the goal of making everybody feel comfortable and safe coming to see the doctor.

Dr. Kharazmi, Physician, Co-Founder
Mohammad S. Kharazmi, M.D., is an assistant clinical professor at the UC Riverside School of Medicine and practices internal medicine at the Riverside Community Hospital where he is also the medical director of the RCH Hospitalist Service. He came to UCR from Eisenhower Medical Center, where he also served as the assistant to the director and taught internal medicine during the hospitalist rotation.
Kharazmi is board-certified in internal medicine and a member of the Societv of Hospital Medicine, the American Academy of Anti-Aging, the American Society of Laser Medicine and the European Hormone Society. He is co-founder of Nugene, a biotechnology company working on growth factors and stem cells.
Kharazmi earned his bachelor's degree in biological sciences from UC Irvine, then got his M.D. from the UC Irvine College of Medicine. He completed his internship and residency at the UCLA-affiliated St. Mary Medical Center in Long Beach and served as chief resident.
Kharazmi has been a co-investigator in developing new nano-encapsulated technology for delivery of growth factors for enhanced wound healing. He is also a co-investigator on an adipose-derived stem cell treatment for osteoarthritis.


George Barboza, Nurse Practitioner
George Barboza is a dedicated Nurse Practitioner committed to providing comprehensive and compassionate care to patients of all ages. He holds an MSN degree and FNP certification from Azusa Pacific University, with extensive experience in diagnosing and treating a wide range of conditions. Known for his warm bedside manner and ability to establish strong relationships with patients and their families, George brings over 24 years of experience in nursing. His expertise has allowed him to develop tailored patient care that resonates deeply with those he serves. George also specializes in advanced wound care and is passionate about providing care to underserved communities.
In his free time, he enjoys spending time with his family, gardening, and playing the guitar.


Osa Ugheighele, Nurse Practitioner
Osa Ugheighele grew up in Nigeria and pursued her initial education at Madonna University, where she earned a Bachelor's degree in Clinical Laboratory Science. Seeking further opportunities, she moved to the United States and attended West Coast University in Ontario, obtaining her Bachelor of Science in Nursing. Osa's extensive clinical experience spans urgent care, ICU, clinics, and hospice care. She furthered her education by becoming a Nurse Practitioner, initially specializing in oncology, and now serving in family practice and skilled nursing facilities. Passionate about patient care, Osa is dedicated to enhancing patient interactions and helping them achieve their healthcare and general wellness goals.


Gabriela Toscano-Lara, Licensed Vocational Nurse
Gabriela Toscano-Lara is a Licensed Vocational Nurse with a passion for helping others and making a positive difference in their lives. With two years of nursing experience and several years in hospice care prior to earning her license, Gabriela brings a compassionate and patient-centered approach to her work. She holds a bachelor’s degree in Biology and Latin American Studies, which enriches her perspective on healthcare and its impact on diverse communities.
Gabriela is inspired by the opportunity to help others, knowing that even small acts of care can improve someone’s life. Outside of work, she enjoys reading, watching movies, traveling, and engaging in creative hobbies like puzzles and building LEGO sets. Her dedication to both her patients and her personal interests highlights her well-rounded approach to life and care.


Hillary Chavarriaga, Registered Nurse
Hillary Chavarriaga is a Registered Nurse with nearly 13 years of experience, including 10 years as a Public Health Certified Nurse. Her background spans Medical-Surgical/Telemetry nursing, acute care case management, and nursing education, where she has trained BSN students in clinical and skills settings.
Inspired by her grandmother’s battle with COPD, Hillary is passionate about providing compassionate, family-centered care. Outside of work, she enjoys camping and exploring new places.


Allison Hanselman, Registered Nurse
Allison Hanselman is a dedicated Registered Nurse with 11 years of experience in healthcare. Known for her compassionate approach, Allison is committed to making a positive impact on her patients’ lives and serving as a trusted source of support throughout their healthcare journeys. Her passion for nursing stems from a deep desire to help others, combining her love for science with her commitment to making a tangible difference in her community.
Outside of her professional role, Allison enjoys spending time with her family, exploring nature, camping, and enjoying music. Her dedication to both her patients and personal well-being reflects her commitment to excellence in care.
